The Community Oak Park (TCOP) collects information about the people in our Community so we can ensure the programs and information we offer align with what is currently important to our Community.

TCOP collects information about our Champions (those who may provide support such a parents, siblings, others) and information about the adults with a developmental disability (Supported Adults) in our Community.

 How does TCOP use the information?  After TCOP has collected the information through Champion and Supported Adult questionnaires, we develop summary reports and analyze the results to develop a clear picture of the current needs, interests and attitudes of our Community. 

This picture helps TCOP:

❏    Design monthly meetings and workshops, choosing topics and speakers that align with your needs and interests.

❏    Develop or choose program offerings (e.g. Cooking and Nutrition, Music, Fitness, Nia and American Sign Language), and others based on your input.

❏    Frame-up a future interest matching method with other Supported Adults or the broader community.

❏    Present the views and advocate for our Community when TCOP interacts with local and provincial government officials and agencies.

❏    Apply for funding for individuals to be engaged, valued and at home in their community.

❏    Make additional resources available such as options for finding a home or inclusion in the broader community.

Information is collected using questionnaire or survey forms.  Here is how we do it:

  1. TCOP first collects basic Contact Information for you (as Champion) and any adults with a developmental disability that you may support (Supported Adult). 

  2. Specific questionnaires for our Champions and Supported Adults are used to gather information to understand the current situation, needs, interests and attitudes. 

  3. TCOP will also send out a Feedback survey every year to help us understand how we are doing and make adjustments.

Ever wonder if your situation is the same or different than others in the Community?  Well, TCOP will present the summary reports and analysis back to the Community so we have a shared understanding.

We recognize that your needs, interests and attitudes may change over time.  About once a year we will reach out and ask if anything has changed.  These are quick updates that should only take 1-2 minutes.  This allows TCOP to shape what it offers to the Community as the needs, interests and attitudes of the Community change.

Being part of a Community is helping each other.  If everybody does their part, we become a stronger Community.  A couple of minutes of your time will go a long way toward helping TCOP help our Community which, in turn, helps you.
