The Community Oak Park Family Group
The Oak Park neighbourhood has apartments, condos and town-homes with very close access to many restaurants, shopping, fitness centres, doctors, dentists, grocery stores, and banks all within minimal walking distance.
The Ministry of Children, Community and Social Services provides funding for eight individuals (The Oak Park Family Group) to live in our community with support in their homes. We are partnered with Community Living Oakville and North Halton to accomplish our goals. With the Community Facilitator they plan a vision for their life and find valued roles in the community.
Individuals live where they want within the community, on their own, or with roommates of their choice and plan their lives with support. The individuals and their families with funding support for Personal Assistants and the Community Facilitator manage their network and support each other.
The eight Oak Park Family Group families all support one another with scheduled formal and social meetings, mom coffee groups and share resources whenever possible. Individuals are integral in planning and creating a life of their own. They make valuable contributions to their community and find roles that are specific to their passions and abilities.
